You are the anaesthetist on call at a regional hospital. As you are walking towards the car park in the evening after a long shift, you hear commotion in the Emergency Department and go to investigate. You find a young man behaving aggressively and screaming abuse at staff in the waiting room. The ED registrar informs you that he appears to be intoxicated with drugs and has amputated his own left arm above the elbow with a large kitchen knife. Both the knife and the amputated arm are currently lying on the floor. Staff have managed to apply a tourniquet to the affected arm but no further intervention has been possible.
